This document summarises the informed consent that every donor signs with Premier Sperm Donors ("PSD").
The consent covers background checks, medical examinations, genetic testing, and the donation process itself.
2. Background and Eligibility
PSD collects information about your family, education, and health history to create an accurate donor profile.
You authorise PSD to complete background checks, including criminal records and education verification.
3. Medical Tests and Examinations
You agree that PSD may conduct the examinations required by law and professional guidelines, such as blood, urine, and genetic testing.
PSD shares results with you so you can follow up with your own physician. PSD does not serve as your personal healthcare provider.
4. Confidentiality and Data Handling
All information is stored securely and accessed only by PSD staff directly involved in the donor programme.
You consent to your data being used for quality assurance, regulatory reporting, and legally required documentation.
5. Biometric Information
PSD may collect biometric data (such as fingerprints or handprints) to verify your identity as part of programme security.
Biometric data is used solely for verification purposes and stored in accordance with current security standards.
6. Right to Withdraw Consent
You may withdraw your consent at any time by contacting PSD. Collected samples can be destroyed upon request where legally permissible.
Certain information may be subject to statutory retention requirements and cannot retroactively be removed.
7. Legal Considerations and Indemnity
You waive any parental rights to children conceived from your donations.
You agree to hold PSD harmless from claims arising out of donations, test results, or sample handling performed in accordance with the consent.
